Grade School Admission Procedure and Requirements
All incoming students must Secure a Grade School Admission Test application form from the Grade School Student Development Center (SDC). Submit the completed application form to the SDC along with the requirements listed below. The testing fees are PhP 250.00 for Grade School and PhP 200.00 for Preschool.
Preschool Age Requirements
Playschool: 3 years old by June 01
Nursery: 4 years old by the June 01
Kindergarten: 5 years old by June 01
Requirements for New Students (Application for Entrance Examination)
1. Authenticated Report Card – Deped form 138* (1 Photocopy)
2. Certificate of Good Moral Character*
3. NSO Authenticated Birth Certificate (1 Photocopy)
4. 2×2 ID Pictures with clear background (2 copies)
5. Accomplished Recommendation Form (For Grades 2 to 6 applicants only)
*Only for applicants with previous schooling
Additional Requirements for Foreign Students
1. Philippine Visa
2. Passport (of parents and applicant)
3. Special Study Permit (SSP)
4. Alien Certificate of Registration (ACR)
5. Transfer records (final grade) with English translations
6. I-Card
Admission Requirements
1. Notice of Acceptance
2. Original Report Card (DepEd Form 138)
3. Original NSO-authenticated birth certificate
4. Original Certificate of Good Moral Character
5. Letter of Intent (Addressed to the Grade School Registrar)
6. 2×2 ID Picture (1 copy)
